Information about your privacy
At the Haarlemmermeer Health Centres Foundation (SGH), your personal data is collected and maintained for the following reasons:
- for the provision of care
- to properly organize our services and improve quality
- for the support of scientific research, education, and information. Regarding the latter point, your data cannot be traced back to you as an individual and is used only for general research purposes.
Your personal data is treated confidentially at the SGH. All employees are bound by professional secrecy and are obliged to handle your personal data confidentially.
If your healthcare provider involves other healthcare providers in your care, your personal data will only be exchanged with your consent, and only to the extent relevant.
Your personal data will not be retained longer than necessary for the provision of good care. For medical data, this retention period is in principle 20 years (from the last treatment). Sometimes longer retention is necessary, for example for the health of yourself or your children.
The SGH has taken appropriate technical and organizational measures to protect your personal data against loss or any form of unlawful processing. Your personal data is well protected against unauthorized access.
Your data will not be used for other purposes and will never be made available to third parties, unless you have given permission to do so.
Right of inspection
You have the right to view your data. You can ask your care provider for access to the part of your file that relates to the care provided by the SGH. If you believe that this data is incorrect, you can request your care provider to correct or delete it. This is also possible if the data itself is correct, but you believe that the recording of that data is unnecessary. Your care provider ultimately determines what is included in the file.
Complaints
If you believe that the SGH is not handling your data correctly, you can file a complaint. The SGH has a complaints procedure. See: complaints.
Camera surveillance
For your safety and ours, the SGH uses camera surveillance. The camera footage is recorded and stored for a certain period. In the event of incidents, the images are used to reconstruct the course of events. The recordings may be transferred to the competent authorities for this purpose. The recorded images are handled in accordance with laws and regulations regarding privacy.
Further questions
If you have any further questions about your privacy within the SGH, you can contact your SGH care provider or send an email to the Data Protection Officer.
