Would you like to register with one of our pharmacies? Welcome!

When filling this out, it is helpful to have the following personal details at hand:

  • Citizen Service Number (BSN)
  • Name of your GP
  • Name of your previous pharmacy

When you visit the pharmacy for the first time after registration, you must identify yourself with a valid form of identification such as an ID card, passport, or driver's license. 

 

Children up to and including 15 years of age can be registered together. We ask adult family members and children aged 16 and over to complete their own form.

 

Do you find it difficult to fill out the form digitally? Don't worry, the registration form is also available at the pharmacy.

Merel van Nimwegen

  • Merel van Nimwegen is a pediatric physiotherapist. She specializes in treating children with headaches, sensory processing issues, and psychosomatic complaints. Since 2025, Merel has been working on Fridays at the Paramedical Treatment Center for Headaches to treat children under the age of 14 with headaches. Obtained a PhD from the Faculty of Medicine at Vrije Universiteit Amsterdam based on research into the effectiveness of manual therapy for tension headache. René supervises and coordinates the collaboration on scientific research into headache between VU Amsterdam and the Paramedical Treatment Center for Headache.
