Registering new patients

New patients welcome:

We are currently accepting new patients. Do you live in the Floriande district or in Zwaanshoek? Then you can register with our GP practice. You can register via the link below:

For your registration, we require the following:

Fill out a separate registration form for each family member
Every family member aged 14 and over must personally submit the registration form to the center assistant.
Bring a valid form of identification (identity card, passport, driver's license, or residence permit).
When submitting, please also bring proof of health insurance (digital, health insurance card, or policy document).

Transfer medical records

After your registration, we ask you to request your previous GP to transfer your medical records to us. This is done via a secure digital connection. However, you must give your consent to your previous GP for this.

The file should also be sent by post to:

If you are receiving treatment at a hospital, would you please have your GP's name changed at the hospital's registration desk? This will ensure that letters from the hospital are sent to the correct GP, guaranteeing that results reach us on time.

No new patients:

Unfortunately, we are currently unable to register new patients and there is no space on the waiting list.
