MijnGezondheid.net

This is the online site where anyone aged 16 and over can arrange various matters with the GP. The website uses DigiD. 

 

When you log in to the portal for the first time, you agree to the terms and conditions. This assures you that the exchange of your data with the practice is properly and reliably arranged.

 

Please note: Do not share your medical records and/or login details with others, even if requested, e.g. by the municipality, employer, or insurance company. If you do wish to share your medical records with others, discuss this with your GP first.

Would you prefer to handle your affairs on your phone or tablet?

This can easily be done with the MedGemak app, part of the patient portal MijnGezondheid.net (MGn). 

 

To use MedGemak, you must first create an account on MGn. With the MedGemak app, you can order repeat medication online, schedule appointments, send messages with attachments, and view your medical record.

 

Here's how it works:

  1. Download the app: available in the [Apple Store] and [Google Play].
  2. Securely link your account to your MGn account with DigiD.
  3. Log in: with your personal 5-digit code.

Here you can make a new appointment, view your upcoming appointments and your appointment history

My medication

Here you can request repeat prescriptions, and view your medications and your medication passport.

Question to the GP / practice nurse (e-consult)

Under 'Ask GP', you can ask your GP or practice nurse a non -urgent medical question . You will receive an answer to your question within three working days. In the absence of your own GP or practice nurse, the e-consultation will be handled by their substitute. The centre assistant reads the messages and adds them to the action list of the GP or practice nurse.

Please note: The e-consultation is not suitable for:
                  – Emergencies (in that case, call the practice)
                  – Problems requiring a physical examination

My file

If you wish to access (parts of) your file, send a request to your GP from MijnGezondheid.net. Your GP will process this request as soon as possible and release your file upon approval.

Report from your GP

Here are the topics and reports that your GP has discussed with you. Your GP has entered this information in your file.

Recorded details

Here you can see sensitivities (for example, whether you are allergic to anything) and details known to your healthcare provider. Do you think this overview is incorrect? Then please contact your healthcare provider.

Measurements from my GP

Here are your measurements from the last 14 months. For example: your blood pressure, if measured by the GP. Your GP has entered these measurements in your file.

Results from your GP

Once your file has been released, the GP can share the results of a laboratory test with you so that you can view the results.

The letters or documents from your GP are here.

Save file

On this page, you can export your data in MijnGezondheid.net to a PDF file. You can then easily print the file. You will also find an overview of your healthcare provider's log data from the past two years on this page, showing when your healthcare provider accessed your medical file.

Here you will find your personal details and, if you wish, add a profile picture.
