Mission and vision
The Haarlemmermeer Health Centres Foundation provides primary healthcare in accordance with applicable national guidelines and standards, with the client's needs at the center.
Vision
The SGH provides effective and safe primary care within its service area. The client has access to the care, both preventive and curative, that is necessary for him or her and falls within the range of services offered by the SGH.
The client can access care within a reasonable timeframe (= in accordance with legal standards and regulations).
The healthcare provider provides the client with relevant information at the client's level. In this way, the client is enabled to make their own decisions regarding the desired treatment (informed consent). The aim is to achieve optimal client involvement.
All clients receive appropriate and correct care regardless of gender, sexual orientation, language proficiency, cultural, ethnic, social, or other background. If necessary, the client is referred to another care provider within or outside the SGH to ensure continuity of care.
Acute primary care is available 7 days a week, 24 hours a day. Authorized, competent, and committed staff are a prerequisite for providing appropriate and correct care. The care provided is evidence-based.
Core values
Care for the client is paramount.
Lines of consultation are short; care providers advise each other and utilize each other's knowledge. The specialist expertise of the care providers is optimally utilized. Collaboration takes place in an open and collegial manner. The care provider, together with the client, maintains control over his or her care, with the client choosing how the care is structured. The SGH participates in networks to optimally coordinate care for clients. Agreements are formalized with chain partners.
