Physiotherapy Health Centres Haarlemmermeer is affiliated with the Onco Care Centre, a network of collaborating BIG-registered disciplines in the region.
The OncoCare Centrum is a primary care oncology center for the Haarlemmermeer, Rijn, Duin & Bollenstreek region, specialized in supporting patients with and after cancer.
Within OncoCare Centrum Hoofddorp, oncological physiotherapy, occupational therapy, dietetics, podiatry, psychologist and the home care organizations Bloezem and Amstelring involved.
Patients face many challenges during the diagnosis, treatment, and aftercare of cancer. For well-coordinated support during this phase of their lives, it is important that they turn to healthcare providers who collaborate effectively and have a good understanding of the condition and the problems they may encounter.
The OncoCare Center strives to provide timely and tailored high-quality care for all indicated (former) oncology patients. The care offered by the OncoCare Center focuses on all functional, physical, psychological, and social problems associated with cancer, at every stage of the disease. This improves the quality of life, as well as the ability to participate in the workforce and society.
An individual care plan is created for each patient, tailored to their needs, (residual) symptoms, limitations, and capabilities. The first healthcare provider the patient contacts within the OncoCare Center is the case manager in the treatment process. The case manager draws up the care plan together with the patient. The plan describes the personal goals and the approach. In cases of treatment involving multiple disciplines, structured multidisciplinary consultation takes place.
