Through our online options, you can manage more and more matters yourself. This means you are not dependent on our opening hours. You have access to your own file via MijnGezondheid.net or the Medgemak app. Many GPs in our region use the same website and app. As a result, some of the information in this digital environment may originate from another healthcare provider, and you may have more options than described below.

Obtaining consent from others

Your privacy is important. That is why we only share your medication or medication details with someone else with your permission. Are you unable to pick up your medication or medication details from the pharmacy yourself? Then you can give someone else permission to do so for you. You can do this using the form that you can fill out online below. However, you can also pick up this form at the pharmacy.

 

You can choose whether to grant permission once or for life. Submit the form to your pharmacy. We will keep it in your file. The person you have given permission to must bring proof of identity when collecting items. Without proof of identity, the pharmacy cannot release your medication or data. Please contact us if you have any questions. 

Due to legislation, this form can be used by clients aged 16 and older. 

For clients aged 12 to 16, the declaration must be signed by both the child and the parent.

Procedure for sending online

  1. Fill in the PDF (Drie Meren)/ PDF (Floriande) online
  2. Save the PDF 
  3. Send the pharmacy a message via the Medgemak app or MijnGezondheid.net and attach the PDF.

You can also print the completed form and hand it in signed at the pharmacy.

Can't see your data? We may not be sharing the data with you yet. In MijnGezondheid.net, click on the 'Settings' tile and then on the 'Data sharing settings' tile. Here, you can request that your medical data be shared with you per pharmacy by clicking on the 'Share data' button.

 

You can access the following data yourself and, if necessary, save and print it:

  • Medication overview, your Current Medication Overview (AMO), for a visit to a doctor
  • Medication passport, for when you go on holiday

What is a medication overview?

A medication overview is a list of all the medicines you use and their dosages. It may also indicate whether you are allergic or hypersensitive to certain medicines. Additionally, it may list medical conditions that the pharmacy takes into account when prescribing medication. These are known as contraindications. 

 

What is a medication overview used for?

Every time your doctor prescribes medication, he must have a medication overview from you. This is to prevent dangerous combinations of medications from occurring. Not all information is shared effectively digitally yet. That is why specialists, in particular, still often ask for a paper overview. 

 

To keep your own medication overview up to date

You play an important role in keeping your medication overview up-to-date and checking it. After all, you are the only one who truly knows which medications you are taking and which you are not. Is something incorrect in the overview? Have you stopped taking a medication, or has the dosage been adjusted by your doctor? Would you please let us know? It is important to keep this overview current.

How do you print these overviews?

Step 1: Log in to MijnGezondheid.net.

 

Step 2: Click on the 'My file' tile.

 

Step 3: You now see an overview of the different sections of your file. Select the section you want to print, for example 'Medication overview' or 'Results from your GP'.

 

Step 4: Once you have selected the desired item, click the pink 'Save as PDF' button. The file will be downloaded, and you can save and print it from your browser.

 

Are you unable to do this yourself? No problem, you can also visit the pharmacy and we will be happy to help you.

Medication passport

Are you going on a trip? Then it is always a good idea to take a medication passport with you. Your medication passport contains all the details about which medicines you use and other matters, such as allergies. Handy if you need to visit the doctor or pharmacy at your travel destination. Please note: the medication passport is not a valid travel document for customs!

 

How do you obtain a medication passport?

You can print a medication passport yourself via MijnGezondheid.net using the Medgemak app.

You can also always stop by the pharmacy, and we will print it out for you. You do not need to call us first for this.

How do you print these overviews?

Do you want to have children? 

At the pharmacy, we would like to know this, not only from women but also from men. We can then check whether any medications currently in use are compatible with the desire to conceive. We will also check whether new medications are compatible with the desire to conceive when a doctor prescribes them. If necessary, we can consult with the prescriber about a potentially safer alternative.

Do not stop taking medication prescribed by your doctor on your own! Always ask your pharmacist or doctor for advice.

 

Are you pregnant?

We would like to be informed early in the pregnancy. It is precisely during the first period that the baby is most vulnerable. If we have not already checked during the planning phase whether the medication can be used, we will do so as soon as we know that someone is pregnant. If necessary, we can consult with the prescriber of the medication about a potentially safer alternative.

 

Are you breastfeeding?

If we were already aware of the pregnancy, we will update this notification to a breastfeeding notification ourselves after the due date. The next time you visit us, we will ask you if you are actually breastfeeding. If we were not yet aware that you were pregnant, we would appreciate it if you would let us know when you start breastfeeding. Medication can also pass to your baby through breast milk. We take this into account and check the medication prescribed by the doctor. If necessary, we can consult with the prescriber about a potentially safer alternative.

 

Are you stopping breastfeeding? We would also like to know that; we will then remove the entry from your file.

You can ask us a question 24/7 via MijnGezondheid.net or the Medgemak app. Please use the messages option for this. On working days, we aim to answer these questions within 24 hours. When you ask your question in this way, the information remains in your file. It is also possible to add an attachment. This is a secure way to share information with us.

How do I send a message to my healthcare provider in MedGemak (or MijnGeondheid.net)?

 

Step 1: Log in to the MedGemak app.

 

Step 2: Click on the “Ask your GP/pharmacy” tile.

 

Step 3: Select the practice/pharmacy to whom you want to send a message.

 

Step 4: Click on “I have a question”.

 

Step 5: Select the healthcare provider to whom you want to send a message.

 

Step 6: Write the subject and your question, add an attachment if necessary, and click the pink button “Proceed to confirmation”.

 

Step 7: Check your composed message and click the pink “Yes, confirm” button to send your message to the healthcare provider. You will now see a notification that your message has been sent.
