Schrijf u dan in via ons patiëntportaal (zie hieronder, inschrijven in patiëntportaal), dan neemt de assistente telefonisch contact met u op binnen 24 uur. Als de assistente uw adres, verzekeringsgegevens en klacht al ontvangt, kan ze uw afspraak goed voorbereiden. Hierna kost het telefonische gesprek minder tijd en kan de assistente snel met u een afspraak maken. 

 

The receptionist will verify your email address and phone number. You will then receive practice information, an appointment reminder, and/or an online questionnaire, possibly via secure email (Zivver). To open the secure email message, you will receive an SMS code. 

 

You will also receive an explanation of the 24-hour policy and your insurance coverage.

The physical therapist will first conduct what is known as a screening. This is to determine whether physical therapy is the right course of treatment for you. The physical therapist will send a report of the screening to your primary care physician. If you object to this, please let us know when you make your appointment. If physical therapy is determined to be the right course of treatment for you, a physical therapy examination will be performed and recommendations will be provided.

If you have supplemental insurance coverage for physical therapy, your health insurer will bill you for two sessions for the first visit.

 

If you do not have supplemental insurance coverage for physical therapy, we will charge you the rate for screening, intake, and examination for your first visit.

 

A follow-up visit, a phone or video consultation, writing a letter, or contacting a doctor or other organization during the treatment period is considered a single treatment by your health insurance provider, and you will be charged for a single session.

Zorg dat u minimaal 24 uur van te voren een afspraak annuleert. Als u de afspraak bij een fysiotherapeut korter dan 24 uur van tevoren annuleert of niet aanwezig bent, brengen wij de gereserveerde tijd in rekening bij u persoonlijk.

 

The 24-hour rule applies even if you have a subscription.

Please bring one large and one small clean towel (for hygiene reasons) and the appointment card you received after your first treatment. If you were referred by a general practitioner outside the health center or by a specialist, please bring the referral with you.

 

We recommend that you download the MijnzorgApp in advance (image showing Windows/Apple download with link). In this app, you can find all the details about your appointment and upload documents to your file.

Physical therapists are required to record the Citizen Service Number (BSN) in their records and to use it when exchanging client information, such as with health insurers and referring physicians. We therefore ask that you bring a valid form of identification (passport, driver’s license, ID card, or alien registration document) with you to your first appointment.

The health center has a privacy policy that ensures your personal information is handled with care. All employees are bound by a duty of confidentiality.

 

We only use secure email (Zivver) to communicate with you. To open the email message, you will receive a text message code.
Through this secure email, you will receive, for example, practice information, appointment reminders, or an online questionnaire. You can also use the Zivver email to ask the physical therapist questions.

 

We also use highly secure systems to communicate with other healthcare providers and agencies.
