This is theonline portalwhere anyone aged 16 or older can handle various matters with their primary care physician. The website uses DigiD.
When you log in to the portal for the first time, you agree to the terms and conditions. This ensures that the exchange of your data with the practice is properly and securely managed.
Please note: Do not share your medical information and/or login credentials with others, even if asked to do so, e.g., by your local government, employer, or insurance company. Would you like to share your medical information with others? Consult your primary care physician first.
Would you prefer to manage your affairs on your phone or tablet?
You can easily do this with the MedGemak app, which is part of the MijnGezondheid.net (MGn) patient portal.
To use MedGemak, you must first create an account on MGn. With the MedGemak app, you can order refills online, schedule appointments, send messages with attachments, and view your medical records.
Here's how it works:
- Download the app: available in the [Apple Store] and [Google Play].
- Securely link your account to your MGn account using DigiD.
- Log in: using your personal 5-digit code.

Question for the family doctor / practice nurse (online consultation)
Under “Ask a Doctor,” you can submit a medical question that is not urgent to your family doctor or practice nurse. You will receive a response to your question within three business days. If your family doctor or practice nurse is unavailable, the e-consultation will be handled by their substitute. The center assistant reads the messages and adds them to the family doctor’s or practice nurse’s to-do list.
Please note: The e-consultation is not suitable for:
– Emergencies (in that case, please call the office)
– Issues that require a physical examination

If you would like to view (parts of) your medical record, please submit a request to your primary care physician via MijnGezondheid.net. Your primary care physician will process your request as soon as possible and release your medical record upon approval.

Here are the topics and reports your primary care physician discussed with you. Your primary care physician has added this information to your medical record.
Recorded details
Here you can see any hypersensitivities (such as allergies) and other details known to your healthcare provider. If you think this information is incorrect, please contact your healthcare provider.

Here are your readings from the past 14 months. For example: your blood pressure, if it was taken by your doctor. Your doctor has entered these readings into your medical record.
