Physiotherapy Practice Regulations
General rules
Treatment takes place upon referral from a general practitioner, sports physician, specialist, or via Direct Access to Physiotherapy (DTF).
FysioFit, 70+Fit, and PilatesFit are individual courses or subscriptions. These subscriptions and courses are non-refundable if you discontinue partway through.
After each (or possibly during) treatment period, a report will be sent to your GP or specialist. If you do not wish this, please inform the treating physiotherapist.
You have the right to access the physiotherapy file at all times.
The Medical Treatment Agreement Act (WBGO) applies to all our treatments.
Consumables are not included in the physiotherapy treatment and are not reimbursed by the health insurer. Therefore, we are obliged to charge these costs to you personally. Such as tape, exercise equipment, etc.
In the event of emergencies, you must follow the instructions of the staff or the competent authorities (fire department, police).
The management accepts no liability for theft or damage to property at the locations of Fysiotherapie Gezondheidscentra Haarlemmermeer or elsewhere.
Smoking is not allowed.
The use of mobile phones is not permitted in the treatment rooms and exercise rooms.
We request that you report defects or malfunctions immediately to one of our employees.
Patients are expected to conduct themselves in accordance with generally accepted norms and values; the management reserves the right to deny access to Fysiotherapie Gezondheidscentra Haarlemmermeer to persons who do not adhere to these.
Use of the exercise room is subject to the house rules of Fysiotherapie Gezondheidscentra Haarlemmermeer:
- Use of the rehearsal room is by appointment only.
- The use of a towel is mandatory.
- Please clean (fitness) equipment after use.
- Bags can be stored in the lockers and are therefore prohibited in the practice room.
Compensation
You are at all times personally responsible for the reimbursement of physiotherapy consultation(s) and therefore for keeping track of the maximum number of treatments reimbursed by your health insurance, as well as for any authorization and/or extension that may need to be requested in advance. If the health insurer does not reimburse the treatment, you must pay the bill yourself. You can view the rates in the waiting room and on the website.
Health insurer agreements
The first appointment is considered by your health insurer as 2 sessions and reimbursed from the supplementary insurance.
Your health insurer encourages us to participate in data analysis. This is done anonymously, and you will not notice it. If you do not want your anonymous data to be shared for research, you can indicate this to the front desk assistant. You can also send an email to fysio@gchaarlemmermeer.nl. We will inform the health insurer that you have objected and will not share your data.
Your health insurer encourages us to ask you to critically evaluate your physiotherapist and the organization. This is done anonymously via an independent agency, Qualiview. Qualiview will email you asking if you would like to complete a satisfaction survey for your physiotherapist. This will take you a maximum of 5 minutes. If you do not want Qualiview to invite you to do this, you can indicate this to the front desk assistant or your physiotherapist. You can also send an email to fysio@gchaarlemmermeer.nl. We will inform the health insurer that you have objected and will not share your data with Qualiview.
Cancelling treatments
If an appointment is cancelled at least 24 hours in advance due to unforeseen circumstances, the reserved time will not be charged.
Appointments that are not cancelled in a timely manner or are not kept will not be reimbursed by the health insurer. Therefore, we will charge the rate for a missed (follow-up) appointment directly to you.
You have the option to convert the appointment into a telephone consultation or a video consultation if you are unable to come to the practice.
Home treatment or video consultation
If you are not insured for (specialized) physiotherapy, a video or telephone consultation is considered a general or specialized session and that rate will be charged. A surcharge applies for home visits. See for details
