Is this your first time visiting us? Or do you have a new complaint?

Please register via our patient portal (see below, register in patient portal), and the assistant will contact you by phone within 24 hours. If the assistant already receives your address, insurance details, and complaint, she can properly prepare for your appointment. This will reduce the time the phone call takes, and the assistant can quickly schedule an appointment with you. 

 

The assistant will verify your email address and phone number. You will then be sent practice information, an appointment reminder, and/or an online questionnaire, possibly via secure email (Zivver). You will be sent an SMS code to open the secure email message. 

 

You will also receive an explanation regarding the 24-hour rule and your insurance.

The physiotherapist will first perform a so-called screening. This determines whether you are in the right place with the physiotherapist. The physiotherapist will send a report of the screening to your GP. If you object to this, please mention this when making the appointment. If you are in the right place with the physiotherapist, a physiotherapeutic examination will be performed and advice provided immediately following the screening.

If you have supplementary insurance for physiotherapy, the health insurer will charge for 2 sessions for the first visit.

 

If you do not have supplementary insurance for physiotherapy, we will charge you the rate for screening, intake, and examination for the first visit.

 

A follow-up visit, a telephone or video consultation, writing a letter, or contact with a doctor or other institution during the treatment period is considered by your health insurer as a single treatment, and one session is charged for this.

Please ensure you cancel an appointment at least 24 hours in advance. If you cancel an appointment with a physiotherapist less than 24 hours in advance orfail to attend, we will charge you personally for the reserved time.

 

The 24-hour rule also applies if you use a subscription.

A large and a small clean towel (for hygiene reasons) and your appointment card, which you received after the first treatment. If you have been referred by a GP outside the health centre or by a specialist, please bring the referral with you

 

We advise you to download the MijnzorgApp in advance (image Windows/ Apple download with link). In this app, you can find all details regarding your appointment and you can also upload documents to your file.

Physiotherapists are required to record the Citizen Service Number (BSN) in their records and to use it for exchanging client data, such as with health insurers and referring physicians. We therefore ask you to bring a valid identification document (passport, driver's license, identity card, or residence permit) to your first treatment.

The health center has a privacy policy that stipulates that your personal data is handled with care. All employees are bound by a duty of confidentiality.

 

We only use secure email (Zivver) for communication with you. You will be sent an SMS code to open the email message.
You will receive, for example, practice information, appointment reminders, or an online questionnaire via this secure email. You can also ask the physiotherapist questions via Zivver email.

 

We also use highly secure systems to communicate with other healthcare providers and agencies.
