When it comes to cancer diagnosis, treatment, and follow-up care, patients face a great deal. To ensure they receive well-coordinated support during this phase of their lives, it is important that they are connected with healthcare providers who work well together and have a thorough understanding of the condition and the challenges they may encounter.
The OncoCare Center strives to provide timely, personalized, high-quality care to all eligible current and former oncology patients. The OncoCare Center’s range of services addresses all functional, physical, psychological, and social issues associated with cancer, at every stage of the disease. This improves quality of life, as well as the ability to participate in the workforce and society.
An individualized care plan is developed for each patient, tailored to their specific needs, (residual) symptoms, limitations, and capabilities. The first healthcare provider the patient interacts with at the OncoCare Center is the case manager for their treatment plan. The case manager develops the care plan together with the patient. The plan outlines the patient’s personal goals and the approach to be taken. When treatment involves multiple disciplines, structured multidisciplinary consultations are held.
